Are you a T-Mobile USA customer? Are you the original purchaser of the device or did you purchase it from Ebay or another third party seller site? Do you and your account meet all of T-Mobile's Device Unlock Policy requirements?

You need to call Tmobile support to iniate the unlock request. No one here on the forums has access to iniate the request.
