Store employee used my account to get a free phone.

When I first enrolled in T-Mobile I did it in person at a local store. Later on after reviewing my account, I found an additional phone that was included initially. I never saw the phone, nor was I ever charged for it. However, if their system is allowing their employees to do that, just imagine what else they get away with.
