Sorry to hear that.
FWIW, I eventually did get my Amazon Fire TV via a coupon code ("rewards code") that I was able to use at checkout on Amazon.com. It's been nearly six months so I don't where I got the code from (e.g., web portal, email).
That said, check your junk mail/spam folders.

Enter all required information, including email, assigned billing number, and Promotional Code: 23FIRETV. You can find your assigned Home Internet billing phone number on your bill under “Connected Devices.”
They said I'd get the code "within 10 weeks," but it was maybe a week or two after that. I purchased T-Mobile Internet after Black Friday to get the deal, and activated the internet device as soon as it came in.
I'd keep bugging T-Mobile on this until you get your TV. Maybe try this number (I found in my call history): 1-888-390-6867.
