Cant find rebate card

I have two $150 rebate credits from T-mobile but I cannot find where to get them. I have both tracking IDs, two promotion numbers and they both say status recieved but I didnt get the card like I have gotten for my keep and switch phone. Anyone know how I can get it? The promo website doesnt show it..
