Hope this helps someone. As I mentioned in my query above, my final bill has negative balance, and I received refund as visa card.

This whole policy is ridiculous. If you cancel an account and have a negative balance, why would you have to request them to refund you money that belongs to you. They should automatically refund you money that belongs to you to your original form of payment. Not only do you have to request your money, but they remove your access to the online account. I too no longer have the PIN so have to drive several miles to a store to I guess prove who I am. Why?? They can refund it to the original form of payment. Worst policy I have ever run across.
