Phone is intermittently switching to "Emergency Calls Only"

So I turned Wi-Fi calling off - I didn't see a preference setting in the Connections menu. It looks like it's still stuck on Emergency Calls Only still with Wi-Fi on, so I guess I'll just leave my phone off Wi-Fi unless I need to be on my internal network for some reason.
One odd note - despite being set to ECO, I can still make outbound phone calls. I called my bank and after about 10-15 seconds the call connected as normal. But my wife tried to call me and it goes straight to voicemail. This is the same regardless of whether Wi-Fi calling is on or off.
