app issue
What do I need to do to see the five devices on the app to show up again when I use a certain phone number because now it only shows one
Nuestra Set online permissions T-Mobile Support page talks about who can do what on the app and website and links you to the settings to update them. Only the Primary Account Holder line can see all lines on the account, so you will want to make sure that you are logging in with that phone number. The other lines on the account will only show their own line and details.

If you mean, the My T-Mobile app for your lines. The number linked to the PAH (Primary Account Holder might have restricted the other numbers from seeing anything outside of their own specific line.
